DUI GROUP REPORTING - FREQUENTLY ASKED QUESTIONS

WHAT DO I BRING TO MY GROUP REPORTING MEETING?
When reporting to DUI Group Reporting, it is necessary to provide your supervising Officer with proof of address and employment (i.e. A piece of mail and your check stub).  When you complete the Live Victim Impact Panel and any court ordered treatment it is also necessary to provide your Officer with those supporting documents.

HOW DO I OBTAIN A TRAVEL PERMIT? 
Traveling out of state requires a permit and must be approved by your supervising Officer.  Travel permits can be obtained during visits to the DUI Group Reporting sites or by reporting to the Adult Probation Offices.  When requesting a travel permit, it is necessary that you provide the dates of your travel, your destination’s address, method of transportation, the purpose of your travel and a phone number where you can be reached while you are out of state.

PLEASE NOTE: If traveling for over two weeks, or leaving the country, it may be necessary to obtain the Judge’s permission.   Please see your supervising Officer for more information.

HOW DO I REGISTER FOR THE LIVE VICTIM IMPACT PANEL AND HOW MUCH DOES IT COST?
The Live Victim Impact Panel costs $40.00.  If you have not yet registered for the Live Victim Impact Panel, please complete the Driver Safety School Court Referral Registration Form.  Once the form is complete you can register one of three ways.

  1. To register by mail send the completed form and payment (personal check, certified bank check, credit card or money order) to Driver Safety School, 111 N. Genesee St. Waukegan, IL 60085.  Once your registration is received you will be notified of the time and date of the Live Victim Impact Panel.
     
  2. To register in person, go to 111 N. Genesee St. Waukegan, IL 60085.  Forms of payment include personal check, certified bank check, credit card or money orders.  NO CASH PAYMENTS ARE ACCEPTED.
     
  3. To register by phone, please call (847)543-2185.  Credit card payment is required when registering by phone.

WHAT SHOULD I DO IF I MISS A GROUP REPORTING DATE?
If you have missed a group reporting date, please refer to the available calendar and report to one of the remaining dates available for that month.  If there are no dates available for that month please contact your supervising Officer immediately.  It is your responsibility to report each month. Josh Hetland (847) 377-3879 and Bret Webster (847) 377-3661.

CAN MY COURT ORDER BE MODIFIED? 
There may be times when your court order may need to be modified.   You can do this by contacting your attorney or filing a motion at the Circuit Clerk’s Office at 18 North County Street Waukegan, IL.  The cost to do this yourself is $40.00.  No personal checks are accepted.

WHEN IS MY NEXT COURT DATE?
You can contact your supervising Officer or the Lake County Circuit Court Clerk at 847-377-3880, to obtain your next court date.

WHERE CAN I COMPLETE MY COURT ORDERED DUI TREATMENT? 
Once completing your DUI evaluation with Nicasa, DUI treatment must be completed with a DASA certified agency service provider.  Once treatment is completed it is your responsibility to provide proof of completion to your supervising Officer.

HOW DO I PAY MY COURT COSTS, SUPERVISION FEES AND FINES?
Your costs, fees and fines must be paid 90 days prior to your termination date.  Payments can be mailed to the Lake County Circuit Court Clerk at 18 N. County Street, Waukegan, IL, 60085.  Forms of payment include certified bank check and money orders.  When mailing in a payment please be sure to include your case number on the certified bank check or money order (NO PERSONAL CHECKS).
 
If paying in person you can utilize any of the Circuit Court Clerk Branches (forms of payment include cash, certified bank check, money orders and credit card, NO PERSONAL CHECKS ACCEPTED) located at:

Please retain your receipts for your records.

What is a BAIID?

WHO DO I CONTACT WITH QUESTIONS ABOUT MY DRIVER’S LICENSE?

WHAT SHOULD I DO IF I GET ARRESTED?
Any new arrest is a violation of your court order and will be reported to the Court.  If you are arrested, you should contact your supervising officer immediately.
